Eligibility

Shelter Plus Care
Funded principally by HUD through NJ State Department of Community Affairs. Targeted but not limited to applicants who are HIV+ and/or diagnosed with a mental illness. Provides a housing voucher (like Section 8), where client pays a portion of their rent. Provides regular case management to help client stay healthy and housed, and link client to needed services
Eligibility Requirements

  • Meets HUD definition of homeless.
  • Physician’s certification of disability
  • Capable of independent living (with case management)
  • Must have an income
  • Agrees to case management and regular meetings

Corpus Christi Ministries Housing
Funded principally by HOPWA, administered by the Jersey City Division of Community Development.  Individuals have a private room in a house shared with four other individuals or a one bedroom apartment. Families have an apartment in a building occupied by other CCMH families. Each participates in case management to aid adherence and health, and help client identify and achieve goals.
Eligibility Requirements
Individual:

  • HIV+
  • Currently homeless (or expecting homelessness upon discharge from an institution)
  • One month “clean time”

Family:

  • One or more family members HIV+
  • Currently homeless
  • One month “clean time”
  • Family size and configuration suitable for available housing

Hudson CASA
Provides case management, including benefits counseling and linkage other needed services. Provides in-house employment specialist and housing specialist to aid client in seeking employment and/or housing.
Eligibility Requirements:

  • Meets HUD definition of “homeless” or is in imminent danger of losing housing
  • Motivated and able to work on their own behalf

RIST
Provides housing and case management services to 25 individuals who are homeless and living with mental illness.
Eligibility Requirements:

  • Meets Definition of Homelessness
  • Living with Severe Persistent Mental Illness
